For an optimal doctor-patient relationship, everyone should feel that it is a “good fit.”

To facilitate your child’s transition to our clinic, you are able to pre-register by clicking below.

After registering, you can then online through our online patient services (link) or by calling 206-780-KIDS.

If possible, please print and sign a copy of:

1. the Bainbridge Pediatrics Medical Records Transfer Letter
   
2. the Bainbridge Pediatrics Consent for Treatment
   

and either fax it to us or drop it off at the clinic and we will work to have your records transferred prior to your first clinic appointment.

Please note it can take 3-4 weeks for record transfers.

Please note that pre-registration will not enroll you as a patient of the practice and does not legally obligate us to provide care until you have established care at our clinic by having an appointment with either Dr. Walters, Dr. Warren or Dr. Hawley at Bainbridge Pediatrics and transferred your medical records.
